Hi,
I could hardly believe it when I stepped on the scale this morning and read 317.75!! It was like a miracle and I was able to picture myself getting below the 300 mark. This program is really working. As I thought about it, I realized that when I get just under the 300 mark I will again be the weight I was at Kellies wedding. Her son is a teenager now, so that is about how long ago that was. I believe he may be about 17, so it was about 18 years ago. I still have the dress I wore then and I will be very happy to wear it again one day.
I must say that I have been contemplating making a slight change based on my experience. I am not sure it is the right thing to do. I dont want to just succumb to my own trick thinking. Here is the problem that I am trying to solve. About two or three days after the cheat meal I begin to highly crave another bit of either high sugar or carb. I am considering adding, not another cheat meal, but one cheat item/serving on day three to carry me past that episode with less stress. I could also simply make it a "floater" and do it when the craving hits but limit it to one item, one serving, one day per week sometime between day one and day six.
I have indulged that one item crave nearly every week, but since that is a deviation from the program I end up feeling guilty and creating a lot of stress for myself. Yet, the program is still working, so it seems plausible that I might be able to simply work in one cheat item on day three (or whenever), simply as a regular part of the program. If it is part of the program I wont have to feel so guilty about it. It often seems to happen on a day that I exercise, too.
I certainly dont want to jeopardize my weight loss progress with my new addition. I just want to smooth things out for myself. I shall try it this week and see how it goes. If, over time, it becomes too hard to manage and I start wanting something every day, I will know that I am being tricked by the addiction. But, if it does not slow my progress and is not too hard to handle I will continue with it. So here goes. Making the change.
Today was day seven of week four and I had my cheat meal of tomato, black bean and hamburger soup with rice on the side. I ended up eating the whole pan of rice which had been made with 1.3 cups of dry rice and 3 cups of water with salt and spices. I set the timer (because of the one-hour time limit) and started with a large bowl of the beans and rice mixed together. It was tasty but something was lacking. I am still not sure what was missing. So then I had two bowls of plain rice with butter -- on the last one I liberally sprinkled some xylitol and cinnamon to make it like a dessert. It was not exactly like rice pudding but it was OK. It is now about 5 hours later and I still feel full. I am not exactly sure what to do with all the left over soup.
I am considering either putting the large jar of soup into the freezer to use at some later time or doling it into plastic bags for smaller portions that would be more convenient to use. I could also just throw it out. If I was not currently suffering with another sinus and bronchial cold, I would give it to some friends but I dont want to share my germs with them. Ill probably just place the jar in the freezer and be done with it.
After a meal like that one I have found that I usually put on about two pounds the next morning but, so far, it has simply dropped off the next day and then the weight loss continues. After a meal like that one, I also find myself ready to return to my nice meals of raw vegetables and protein. They simply make my body feel better.

Soy for a Healthy Liver
Researchers in Mexico found when they fed hyperinsulinemic rats (rats with high blood levels of insulin) diets high in soy protein, it decreased the amount of fat in the rats liver.People with insulin resistance or diabetes are more likely to have higher levels of insulin in their blood. That insulin can lead to increased production of fatty acids in the liver leading to more pockets of fat in liver cells (hepatocytes).
A fatty liver by itself doesnt damage liver cells. But its sometimes associated with liver inflammation, which can be damaging. The Liver Foundation describes the causes and treatment of fatty liver disease.
How did soy protein protect against fat buildup in the rats liver? It reduced the expression of certain genes responsible for fat production. It was also capable of increasing the breakdown of fat.

2 cups of milk a day ideal for childrens health new research shows
New research has answered one of the most common questions parents ask their doctors: How much milk should I be giving my children? The answer is two cups per day.
"We started to research the question because professional recommendations around milk intake were unclear and doctors and parents were seeking answers," said Dr. Jonathon Maguire, a paediatrician at St. Michaels Hospital and the lead author of the study.
Dr. Maguire and his team looked at how cows milk affected body stores of iron and vitamin D – two of the most important nutrients in milk – in more than 1,300 children aged two to five years.
The results of the study appeared online in Pediatrics today.
They found that children who drank more cows milk had higher Vitamin D stores but lower iron stores.
"We saw that two cups of cows milk per day was enough to maintain adequate vitamin D levels for most children, while also maintaining iron stores. With additional cows milk, there was a further reduction in iron stores without greater benefit from vitamin D," Dr. Maguire said.
The researchers recruited healthy children during routine doctors appointments between 2008 and 2010. Parents were asked to fill out an extensive questionnaire about their childrens milk drinking habits and other factors that could affect iron and Vitamin D stores. A blood sample was obtained from each child to determine body stores of iron and Vitamin D.
The children were participating in TARGet Kids!, a unique collaboration between childrens doctors and researchers from St. Michaels Hospital and The Hospital for Sick Children. The program follows children from birth with the aim of understanding and preventing common nutrition problems in the early years and their impact on health and disease later in life.
The study also suggested that children with darker skin pigmentation may not have enough vitamin D stores during the winter months. Dr. Maguire suggested that instead of consuming more milk to increase these levels, wintertime vitamin D supplementation may be a more appropriate way of increasing vitamin D stores while preserving iron stores.
"Vitamin D deficiency in children has been linked to bone health issues and iron deficiency has been linked to anemia and delays in cognitive development," Dr. Maguire said. "Being able to answer parents questions about healthy cows milk intake is important to avoiding these potentially serious complications of low vitamin D and iron stores."

Health Insurance Rebates A 1 3 Billion Non Event
The Disease Management Care Blog has a lingering weakness for garden statuary. It thinks that nothing quite compliments phlox and snapdragons better than a weenie cherub or squatting amphibian. The skeptical DMCB spouse disagrees. Tired of kneeling trolls disturbing her caully laid mulch, she questions whether her husbands admiration for toadstools, rabbits or fairies projects the right level of curbside sophistication.
Skepticism may also be a reaction of 16 million Americans who are about to discover a health insurance garden gnome rebate check in their mailbox. According to The Hill, the commercial insurers that havent met the Affordable Care Acts 80 to 85 percent medical loss ratio rule will have to give back a total of $1.3 billion in premium rebates this summer. Whats more, the checks will have to be accompanied by a statement that theyre the result of Affordable Care Act.
The DMCB thinks this will turn out to be a non-event for four reasons:
1) What Does This Really Mean to an Individual?
When the DMCB does the math, $1.3 billion spread over 16 million eligible Americans works out to $81.25. This article says the amount will be higher at $127. While the DMCB thinks any free cash is always welcome, its inner behavioral economist doesnt think a check for around a hundred bucks is going to generate much buzz among health care consumers, especially among a population segment that can already afford to buy commercial insurance.
2) Remember the Tax Rebate Checks of 2008?
Neither can the DMCB. But tracking polls at the time suggested they did little to capture Americas imagination or boost consumer confidence. That doesnt mean that Washington DC doesnt continue believe that giving something for nothing will generate gratitude and votes. All the better if its done just prior to a major election and humiliates health insurers.
3) Speaking of Which....
Why arent the health insurers pushing back by pointing out that the rebates pale next to the cost of the ACAs mandates and that a one-size-fits-all medical loss ratio rule is unfair to the individual insurance market? Could it have something to do with $1 trillion in new business or the threat of being resurrected as an anti-progressive bogeyman? The DMCB calculates that $1.3 billion will be viewed by consumers as the price of doing business with a hostile federal government.
4) Plus, Is A Billion A Lot? Really?
The DMCB looks forward to hearing our chattering politicians extol the rebate of "$1.3 billion." While that used to sound like a lot, our unending budget fiasco has since taught Americans that the term "billion" is now synonymous with a federal budget rounding error. Who can forget Harry Reids quote that the billions saved in passing tort orm was insignificant?
Next stop for the DMCB will be a bunch of solar powered lamps and a volleyball-sized mirrored gazing ball. What the Administration has planned for our health insurers is another matter.

Excess vitamin E intake not a health concern
Despite concerns that have been expressed about possible health risks from high intake of vitamin E, a new review concludes that biological mechanisms exist to routinely eliminate excess levels of the vitamin, and they make it almost impossible to take a harmful amount.
No level of vitamin E in the diet or from any normal use of supplements should be a concern, according to an expert from the Linus Pauling Institute at Oregon State University. The review was just published in the Journal of Lipid Research.
“I believe that past studies which have alleged adverse consequences from vitamin E have misinterpreted the data,” said Maret Traber, an internationally recognized expert on this micronutrient and professor in the OSU College of Public Health and Human Sciences.
“Taking too much vitamin E is not the real concern,” Traber said. “A much more important issue is that more than 90 percent of people in the U.S. have inadequate levels of vitamin E in their diet.”
Vitamin E is an antioxidant and a very important nutrient for proper function of many organs, nerves and muscles, and is also an anticoagulant that can reduce blood clotting. It can be found in oils, meat and some other foods, but is often consumed at inadequate dietary levels, especially with increasing emphasis on low-fat diets.
In the review of how vitamin E is metabolized, researchers have found that two major systems in the liver work to control the level of vitamin E in the body, and they routinely excrete excessive amounts. Very high intakes achieved with supplementation only succeed in doubling the tissue levels of vitamin E, which is not harmful.
“Toxic levels of vitamin E in the body simply do not occur,” Traber said. “Unlike some other fat-soluble vitamins such as vitamins A and D, it’s not possible for toxic levels of vitamin E to accumulate in the liver or other tissues.”
Vitamin E, because of its interaction with vitamin K, can cause some increase in bleeding, research has shown. But no research has found this poses a health risk.
On the other hand, vitamin E performs many critical roles in optimum health. It protects polyunsaturated fatty acids from oxidizing, may help protect other essential lipids, and has been studied for possible value in many degenerative diseases. Higher than normal intake levels may be needed for some people who have certain health problems, and smoking has also been shown to deplete vitamin E levels.
Traber said she recommends taking a daily multivitamin that has the full RDA of vitamin E, along with consuming a healthy and balanced diet.

A Definition of Big Data for Health Care Providers and Five Useful Caveats

Written from a "business intelligence" perspective, there were precious few insights for the population and care management community. Were aware of the concept, but how, asks the Disease Management Care Blog, does it apply to our corner of the health care delivery system?
Unable to resist, the DMCB donned its snorkle and flippers and took a deep dive at the topic.
First off, when the DMCB performed a classic medical literature search using the key words "big data," it found that that the term has not entered the health care lexicon in a big way. Academics instead per to write about "registries," "data warehousing" and "predictive modeling." The DMCB also looked for a standard health care definition of "big data" and could find none in the published medical literature.
So, the DMCB offers up its own definition, culled from papers like this and this:
Health care "big data" is a branch of health care informatics that pools large and disparate data sets and applies a suite of mathematical approaches that derives associations, facilitates comparisons and generates insights that are otherwise not possible using standard mono-source analytics. It includes, but is not limited to, reporting, dashboards, ad-hoc queries, graphical displays, scorecards, predictive modeling, data mining and business intelligence. The data sets can be comprised of EHR data, insurance claims, pharmacy utilization, care management systems, consumer as well as government information, public health, surveys, point-of-contact information and web-usage.
The DMCBs simplistic off-the-cuff examples of big data queries include examining 1) the association between "hits" from a cluster of ISPs on an emergency rooms web page and ER utilization, 2) complaints about a hospitals food service from family and the likelihood of being named in a malpractice suit, 3) looking for rare side effects among persons with a cluster of medical diagnoses who are using a just-released drug and 4) whether the number of household flat screens is a useful predictor of obesity.
Five DMCB caveats:
1. One data integrity trumps five Ph.Ds: The chief challenge is not the mathematics but combining and aligning the various databases. Once the information is teed up, its amazing how much can be done by a masters-level statistician and a desktop PC.
2. Associations, not causality. Whether a web page leads to ER visits or whether bad food fuels dissatisfaction is a different question. Its possible that ER visits prompt web usage or that already dissatisfied patients find overcooked string beans icky. All the possibilities are still useful insights.
3. Not a panacea: Its "a" tool, not "the" tool. Users will still need to also invest in faster, better and cheaper mundane data tasks (like admissions per thousand) while they simultaneously understand how big datas associations, comparisons and insights generate additional patient value.
4. Journey, not destination: Theres a potent mix of art, science and wizardry in the evolving science of "big data." There are no standard methodologies or best practices. Get used to it.
5. Skepticism abounds: Data stakeholders who are used to standard analytics will er, as the DMCB found out, to "big data" as "voodoo," and resist buy-in. If a critical mass of an organizations leadership comes to believe its useful, the rest will follow... eventually.

Another Reason Why There Was A Decline In U S Hospitalizations for Heart Failure
By now, many Disease Management Care Blog readers have become aware of this JAMA research study that used Medicare fee-for-service claims data to examine the nationwide rate of hospitalizations for chronic heart failure. From 1998 to 2008, there was a counterintuitive 30% decline in the U.S. from a baseline rate of 2845 to a new rate of 2007 admissions per 100,000 person-years. The authors credit better care of heart attacks (damage from a heart attack to can lead to a flabby dilated heart), better prevention (such as more aggressive treatment of high blood pressure, which also causes heart damage) and a more "effective" medical system (such as better outpatient follow-up, use of alternate levels of care, flu shots prescriptions of ACE inhibitors and beta blocker medications). The authors of the study think the numbers are remarkable because the U.S. population is getting older and healthier persons (without heart failure) seem to be signing up for managed care Medicare Advantage.
The DMCB is wondering about another possibility that has nothing to do with epidemiology or quality. Rather, it could be the impact of Medicare payment rates on billing patterns. After all, if heart failure is the leading Medicare inpatient diagnosis, shouldnt a decrease there have an impact on the overall hospitalization rate?
The DMCB explains:
When beneficiaries are discharged from a hospital, the bill (or the claim) submitted to Medicare is based on a "Diagnosis Related Group." While the invention and logic of DRGs complicated, theyre important because the principal diagnosis determines the amount of the global payment for that hospitalization. While this may be overly simplistic, a discharge with a diagnosis of "heart failure" prompts Medicare to pay a hospital "X" dollars, while a diagnosis of pneumonia or kidney failure will render payments of "Y" and "Z" dollars, respectively. In general, the more complicated the diagnosis, the greater the payment.
All well and good, but suppose the hospital has a patient with several concurrent problems and has a choice on which DRG to use? As anyone who has taken care of hospitalized patients knows, there are usually multiple diagnoses present in any patient at one time. Pneumonia may or may not have provoked the heart attack that led to the kidney failure that led to the leg swelling and the shortness of breath. Given three simultaneous diagnoses, Medicare billing guidelines state that the hospital should use their best judgement to determine which DRG to bill. All things being equal, smart hospitals will probably use the DRG that renders the greatest payment.
The DMCB isnt saying that fraudulent billing (for example) accounts for the decrease in heart failure hospitalizations for Medicare. However, it knows some diagnosis related groups can be less remunerative than others and that in the last ten years, DRG payment rates have evolved and that hospitals have learned how to "code" more accurately and aggressively. Based on the example at the bottom of this page, the DMCB wonders if some patients that were diagnosed with heart failure in 1988 would have been diagnosed with something more remunerative in 2007.
In other words, there may have been the same number of hospitalizations involving the same patients with the same disease burden. It was the case mix that changed?
The authors of the study to their credit cant discount the possibility: They argue that if coding had changed there would have been a shift in the mortality rate of patients with heart failure:
"We were unable to determine whether the observed changes were due to changes by hospitals in medical coding; however, substantial up-coding or down-coding would likely result in changes to the coefficients of the CMS HF mortality model, and these coefficients remained stable from 2005 to 2008."
The authors may have a point, but that assumes the modeling - also based on claims - is trustworthy.
Of course, there is no way, based on Medicare billing claims alone, to determine whether measurement also played a role in the decline in heart failure admissions. That would take an audit of the medical records themselves.

Thats when the DMCB excitedly pointed out that Facebooks launch has many lessons, including the overlaps between hype and greed, the wackiness of giving money to a 28 year old hoodie-wearing oddball and the certainty that if everyone could only "friend" everyone, cheap solar power would finally prevail. Inspired by the luster of a multi-billion dollar IPO, it thinks now is the time to cash out the retirement funds, leverage the DMCBs brand and launch its own social media offering called "Woebook."
The User Profile: This will list any and all medical conditions that may lect a users, family members or friends medical history, or a professional interest or a passing hobby. If you dont know your condition, a partnership with IBMs "Watson" will assign one to you. If you have no condition, a bank of specialists will be on call to make one up for you.
Friends: Communities of the Diseased will spring up, share symptoms and insights and give special meaning to the term "poked."
Marketing: Instead of relying of some claptrap on how targeted and intrusive ads somehow "enrich" the users "experience," Woebook will be honest and announce its intent to bludgeon users with what they really want: information on pricey brand name drugs and the latest and most expensive medical devices.
Privacy Settings: While the default is HIPAA-level privacy and the threat of unending visits by U.S. Department of Justice bullies, Woebook will maintain a jumbotron in Silicon Valley that will rotate the pages of users who have selected the "maximally voyeur" privacy setting. Banner ads scrolling across the bottom will generate tons of advertising revenue. Best of all, those annoying Millennials in their BMWs will be reminded that something incurable is waiting them just a few years away.
Speaking of Ads: Youll have to give permission to have them appear on your pages. If you accept that option, the DMCBs Woebook will share its vast ad income with you. Users will also have the option of sanctioning spurious, malicious or uncool ads and, if enough votes are collected, the ad and the sponsoring company will be banned. That includes for profits, not-for-profits, government agencies and candidates for U.S President.
Photo sharing: You bet, including but not limited to that unsightly and tough-to-diagnose rash or non-healing sore, x-ray images and scans of the mysterious "this is not a bill" documentation.

What to Expect During a Well Baby Check Up
During the first year of a babys life, the pediatrician will carefully monitor everything about them. This is to make sure that your baby is growing properly and getting the right kinds of nutrition. During a well-baby check-up the doctor will carefully examine your baby from head to toe. These doctors visits continue throughout the first two years starting at every two weeks then four weeks then after a few months. Being diligent about having your baby examined by their doctor will help alleviate any medical surprises later on.
First the doctor will take your childs measurements. Tracking height and weight during the first year is helpful to doctors and parents. It helps measure your babys growth and also identify if there is a problem with their growth that may not be manifesting through other symptoms. Additionally, the pediatrician will make sure that there is no fluid build in up in the ears which can lead to ear infections or other problems. In the mouth, the doctor will be checking for signs of thrush. Finally the physician will inspect the baby for bruises, rashes, birthmarks and jaundice. There are many indications on a babys skin that can be related to a medical disorder. It is very important that at home you monitor any birthmarks or rashes and take the child to the doctors if they get worse or spread.
Caring for a child is a lifelong commitment but the hardest period is the first year of their life. There are hundreds of disorders and medical conditions that a baby can develop throughout the first 12 to 18 months. For this reason it is important that parents are diligent in taking their child to the doctors office and having them perform routine examinations. Most parents have nothing to worry about. As long as you are maintaining the best care possible at home, your child will likely grow up to be happy and healthy.
