Diabetes Link To Bacteria

Diabetes is often accompanied by chronic low-grade inflammation.  Recent research indicates that inflammation may be linked to molecules derived from bacteria.

It has been hypothesized that bacterial endotoxins, lipopolysaccharides (LPS) located on the outer membrane of Gram-negative bacteria, can increase the risk for type 2 diabetes by activating components of the immune system which promote insulin resistance and other metabolic abnormalities.

In this large prospective study from last year:

Endotoxemia was associated with an increased risk for type 2 diabetes. Notably, the increased risk was independent of established diabetes risk factors such as blood glucose, serum lipids, C-reactive protein (CRP), and body mass index (BMI). The risk was also independent of factors known to affect serum endotoxin activity, such as total cholesterol, triglyceride levels, HDL cholesterol levels, and smoking.
"Our results indicate for the first time that endotoxemia is a key player in the pathogenesis of diabetes and that microbes may have a central role."

High Fat Intake Leads to Acute Postprandial Exposure to Circulating Endotoxin in Type 2 Diabetic Subjects, Diabetes Care, December 2011

Found that a high-fat meal elevated levels of endotoxins, especially in people with diabetes.
"These studies have highlighted that exposure to a high-fat meal elevates circulating endotoxin irrespective of metabolic state, as early as 1 h after a meal. However, this increase is substantial in IGT and type 2 diabetic subjects, suggesting that metabolic endotoxinemia is exacerbated after high-fat intake.

In conclusion, our data suggest that, in a compromised metabolic state such as type 2 diabetes, a continual snacking routine will cumulatively promote their condition more rapidly than in other individuals because of the greater exposure to endotoxin."
Previous studies implicated dietary fat in levels of endotoxins (LPS):
"In mice fed a high-carbohydrate diet, the increase in plasma LPS was blunted compared with mice fed a high-fat diet."

"In this large sample of healthy men from a population-based sample, we found a link between food intake and plasma LPS. Experimental data suggest that fat was more efficient in transporting bacterial LPS from the gut lumen into the bloodstream."

How does dietary fat assist endotoxin absorption?  Its been proposed that fat-soluble endotoxins mix with and are absorbed in conjunction with the fat contained in a meal:
"Dietary fat is incorporated from the gut into triglyceride-enriched lipoproteins, chylomicrons, whose formation promotes LPS absorption."

Another hypothesis is that the absorption of endotoxins is assisted by inflammation present in the gut.  This novel study found that orange juice, taken with a meal, could lower levels of circulating endotoxins:
"Whether this increase of endotoxemia is due to the lipid solubility of endotoxin and its absorption into the circulation with the fat contained in the meal or is secondary to other factors such as the inflammation of the intestinal epithelium is not clear. If it is secondary to other factors, the potential antiinflammatory effect of orange juice intake could lower postprandial endotoxin increase."

The following study found a strong correlation between endotoxins and features of the Metabolic Syndrome (in line with the findings of the study at the top of this post which found an association between endotoxins and type 2 diabetes):

Bacterial Endotoxin Activity in Human Serum Is Associated With Dyslipidemia, Insulin Resistance, Obesity, and Chronic Inflammation, Diabetes Care, August 201

It is known that HDL cholesterol assists in the elimination of endotoxin from the circulation.  (In healthy individuals, endotoxins are bound mainly to HDL).  This study found that diabetic patients with low HDL often had high endotoxin activity.
"Of all the tested clinical variables, the strongest correlation was observed between the LPS/HDL ratio and serum triglyceride concentrations.  High fasting concentrations of triglycerides predict postprandial hypertriglyceridemia and the development of insulin resistance."
The study also found an inverse correlation between serum LPS activity and insulin sentitivity in both diabetic and nondiabetic groups.  High endotoxin activity was linked to insulin resistance.

The contribution of a high-fat diet was also noted:
"Human and animal studies have both highlighted the importance of the composition of the diet for its influence on the endotoxin absorption process. Consumption of an energy-rich high-fat diet may result in increased levels of gut-derived bacterial endotoxins in circulation."
The authors concluded:
"Taken together, these data show that endotoxins derived from gram-negative bacteria are strongly associated with the MetS variables in vivo. These observations may have clinical implications, because high LPS activity is more often found in subjects who show signs of dyslipidemia, insulin resistance, overweight, and inflammation—factors that increase the risk for diverse micro- and macrovascular complications."

Increasing Red Meat Consumption Increases Risk For Type 2 Diabetes In 3 Large Cohorts

Following the theme of yesterdays post, Large Prospective Study Supports Link Between Meat Consumption And Diabetes, below is a study from this summer which found that increasing the amount of red meat eaten - by just a half serving or a few ounces a day - increases the risk for type 2 diabetes:

Changes in Red Meat Consumption and Subsequent Risk of Type 2 Diabetes Mellitus: Three Cohorts of US Men and Women, JAMA Internal Medicine, July 2013

Researchers followed 149,143 participants of 3 Harvard-based prospective cohorts - the Health Professionals Follow-Up Study, the Nurses’ Health Study, and the Nurses’ Health Study II. After adjustment (for age, family history, race, marital status, initial red meat consumption, smoking status, physical activity, alcohol intake, total energy intake, and diet quality), they found that:
"Increasing red meat intake during a 4-year interval was associated with an elevated risk of [type 2 diabetes] during the subsequent 4 years in each cohort."

"Increasing red meat intake of more than 0.50 servings per day was associated with a 48% elevated risk [of type 2 diabetes] in the subsequent 4-year period.

"Reducing red meat consumption by more than 0.50 servings per day from baseline to the first 4 years of follow-up was associated with a 14% lower risk during the subsequent entire follow-up."
They concluded:
"Our results confirm the robustness of the association between red meat and [type 2 diabetes] and add further evidence that limiting red meat consumption over time confers benefits for [type 2 diabetes] prevention."
People change their eating behavior over time. Studies that measure food intake at one point in time, usually at baseline or the beginning of a study, fail to account for these changes. In these 3 cohorts, diet was updated every 4 years.

Importance of Diabetes Diet and Blood Sugar Testing Strips in a Diabetic’s Life

Morning
6 a.m. – Take ½ teaspoon fenugreek powder + water.
7 a.m. – A cup of sugar free tea and a mary biscuit.
8.30 a.m. A plate upma or oatmeal and half bowl sprouted grains. Have a cup of low fat milk without sugar
10.30 a.m. A fruit or low fat buttermilk, or lemon water
Afternoon
1 p.m. - 2 roti of mixed flour, a bowl of lentil, a bowl of yogurt, a bowl of vegetable, and a plate of salad.
Evening
4 pm - A cup of tea without sugar and a toast or a biscuit.
6 pm – A cup of soup
Night
8.30 pm - 2 roti of mixed flour, a bowl pulse, half bowl green vegetable, and a plate of salad.
10.30 pm – A cup of milk without sugar.
It is important to strictly avoid sugar, molasses, honey, sweets, and dry fruits. It is important to include a light exercise regime daily. For instance, you can go for 35-40 minute brisk walk daily. Also avoid oily food and increase the intake of fiber foods in meals.

PCB Exposure May Increase Risk For Diabetes

In light of the recent contamination of pork in Ireland (On Saturday, Ireland recalled all pork products because they were contaminated with high levels of PCBs. PCBs are polychlorinated biphenyls, highly toxic petroleum products. Yesterday, the Irish government reported that cattle were also testing positive for dioxins), I bring the unfortunate news that exposure to PBCs can increase the risk for diabetes.

In 1978-1979, people in central Taiwan were poisoned by PCB-contaminated rice bran oil that they used for cooking. Between 1993 and 2003, researchers in Taiwan investigated the incidence of type 2 diabetes in these Yucheng ("oil disease") victims.

When compared to non-poisoned controls, women who were exposed to the dioxin-laced oil were twice as likely to have developed diabetes in the ensuing 24 years.

For women whose PCB exposure was greatest (as evidenced by the presence of chloracne - an acne-like skin disorder caused by exposure to toxic chlorinated chemicals), diabetes risk was five and a half times that of matched controls (hypertension risk was 3.5 times that of matched controls).

Men exposed to dioxin also experienced an increased risk for diabetes but it was not significant. The researchers speculated that "women tend to have a greater fat percentage than men, which might result in a longer half-life of these lipophilic compounds."

One of the studys authors, Dr. Yueliang Leon Guo, commented to Reuters Health on the implications of their findings:
"Guo said that since "everyone" has detectable PCB levels in his or her body, its possible that exposure to such pollutants has helped feed the widespread rise in diabetes in recent decades."
The authors advised:
"When planning treatments against diabetes, the body burden of PCBs and dioxins should be caully considered, especially for women."

Increasing Muscle Mass May Improve Insulin Resistance And Lower Risk For Diabetes

Is it possible to improve insulin sensitivity and stave off a diagnosis of type 2 diabetes by increasing muscle mass?

Yes, according to this study:

Relative Muscle Mass Is Inversely Associated With Insulin Resistance And Prediabetes. Findings From The Third National Health And Nutrition Examination Survey, The Journal of Clinical Endocrinology and Metabolism, September, 2011

Analysis of data from 13,644 participants of NHANES III revealed:

In people with diabetes, each 10% increase in skeletal muscle index (SMI: the ratio of total skeletal muscle mass, estimated by bioelectrical impedance, to total body weight) was associated with:
  • 11% reduction in insulin resistance (HOMA-IR)
  • 12% reduction in prediabetes prevalence
Reductions were higher in people without diabetes.

Study author Dr. Preethi Srikanthan, an assistant professor of medicine in the division of endocrinology at UCLA, said in a press release:
"Our findings suggest that beyond focusing on losing weight to improve metabolic health, there may be a role for maintaining fitness and building muscle mass. This is a welcome message for many overweight patients who experience difficulty in achieving weight loss, as any effort to get moving and keep fit should be seen as laudable and contributing to metabolic change."

Eating more red meat associated with increased risk of Type 2 diabetes


Eating more red meat over time is associated with an increased risk of type-2 diabetes mellitus (T2DM) in a follow-up of three studies of about 149,000 U.S. men and women, according to a report published Online First by JAMA Internal Medicine, a JAMA Network publication.

Red meat consumption has been consistently related to an increased risk of T2DM, but previous studies measured red meat consumption at a baseline with limited follow-up information. However, a persons eating behavior changes over time and measurement of consumption at a single point in time does not capture the variability of intake during follow-up, the authors note in the study background.

An Pan, Ph.D., of the National University of Singapore, and colleagues analyzed data from three Harvard group studies and followed up 26,357 men in the Health Professionals Follow-up Study; 48,709 women in the Nurses Health Study; and 74,077 women in the Nurses Health Study II. Diets were assessed using food frequency questionnaires.

During more than 1.9 million person-years of follow-up, researchers documented 7,540 incident cases of T2DM.

"Increasing red meat intake during a four-year interval was associated with an elevated risk of T2DM during the subsequent four years in each cohort," according to the study.

The results indicate that compared with a group with no change in red meat intake, increasing red meat intake of more than 0.50 servings per day was associated with a 48 percent elevated risk in the subsequent four-year period. Reducing red meat consumption by more than 0.50 servings per day from baseline to the first four years of follow-up was associated with a 14 percent lower risk during the subsequent entire follow-up.

The authors note the study is observational so causality cannot be inferred.

"Our results confirm the robustness of the association between red meat and T2DM and add further evidence that limiting red meat consumption over time confers benefits for T2DM prevention," the authors conclude.

Commentary: Oxygen-Carrying Proteins in Meat and Risk of Diabetes Mellitus

In an invited commentary, William J. Evans, Ph.D., of GlaxoSmithKline and Duke University, Durham, N.C., writes: "The article by Pan et al confirms previous observations that the consumption of so-called red meat is associated with an increased risk of type 2 diabetes mellitus (T2DM)."

"Perhaps a better description of the characteristics of the meat consumed with the greatest effect on risk is the saturated fatty acid (SFA) content rather than the amount of oxygen-carrying proteins," Evans continues.

"A recommendation to consume less red meat may help to reduce the epidemic of T2DM. However, the overwhelming preponderance of molecular, cellular, clinical and epidemiological evidence suggests that public health messages should be directed toward the consumption of high-quality protein that is low in total and saturated fat. … These public health recommendations should include cuts of red meat that are also low in fat, along with fish, poultry and low-fat dairy products. It is not the type of protein (or meat) that is the problem: it is the type of fat," Evans concludes.

Like Eat Apples Diabetes Away Apple Benefits

Like Eat Apples, Diabetes Away | Apple Benefits | Recent research from the United States found that the consumption of some types of fruits such as blueberries, apples and pears associated with lower risk of diabetes.

According to researchers, this fruit contains flavonoids, naturally occurring compounds present in fruits, vegetables and grains, which is based on the results of some research related to health benefits such as lower risk of heart disease or cancer.

"People who eat blueberries or apples in a higher number, they have a lower risk of type 2 diabetes,"
said An Pan, a researcher from the Harvard School of Public Health.

According to Pan, the findings suggest an association, but does not directly prove that the fruits could prevent diabetes.

Research published in the American Journal of Clinical Nutrition showed results consistent with some previous research which had already connect a diet high in fruits (rich in flavonoids) associated with a decreased risk of hypertension.

According to the American Diabetes Association, approximately 26 million Americans have diabetes. This is caused by the bodys inability to produce or use insulin, a hormone that converts glucose in the blood into energy.

Type 2 diabetes can usually be controlled with exercise and dietary changes and without insulin.

In the study, researchers from the U.S. National Institutes of Health track the diet of some 200,000 men and women for 24 years. The participants, are requested to fill out questionnaires about how often they eat certain foods and beverages with standard portion sizes. In early research, all participants are known no one had diabetes, but during the study period, a total of 12 600 participants were diagnosed with diabetes.

Research results found that those who love the blueberry has a 23 percent lower risk of type 2 diabetes, compared with those who do not eat blueberries. While participants who ate five or more apples per week also had 23 percent lower risk of developing diabetes than participants who did not eat apples.

The researchers revealed that the flavonoids in fruits may have beneficial effects in reducing the risk of diabetes. "We found consistent results from three research results. Where the three studies indicate that consumption of apples and blueberries beneficial for preventing type 2 diabetes," said Pan.

According to Pan, the findings took into account other risk factors, such as weight, smoking and family history of diabetes.
