Can Our Politicians or the Market Improve on NICE

In the halcyon dog days of summer 2009, U.S. politicians aimed their rhetorical fire on a rather odd target: the UK’s National Health Service (NHS).  With a vote on President Obama’s proposed health orm on the horizon, politicians hoped to tar the orm effort by association: The NHS was painted as inefficient; a decried relic of “socialized medicine” and state planning; some damning charges were levied against NICE (The National Institute for Clinical Excellence) as the putative “death panel” rationing potential life-saving treatments to patients.

In a strange turn of events, recent developments in implementing the ACA, particularly the legislative appropriation of basic benefits, may show the limits of the US political system in making important decisions for patients in an economically efficient matter.
 
Two recent examples may raise red flags.  ACA’s “Basic Benefit” program, originally conceptualized as a way for the federal government to standardize benefit programs offered on health care exchanges, was devolved to the states in order to dispel concerns over excessive bureaucratic intervention in the process. 

The result: massive lobbying by medical interest groups, including chiropractors and acupuncturists, to include their services in the benefit package without examination of medical evidence or economic value of their use. While these rent-seeking activities were seen as part and parcel of the political process, the Nebraska basic benefits proposal to offer a high-deductible plan did not pass muster. 

One economic case for the ACA (albeit a limited one) was predicated on leveraging efficiencies in drug prescriptions for generic drugs, particularly biosimilars.  Recent reports indicate, however, that biotechnology companies are trying to put additional roadblocks in the process to prescribe biosimilars, perhaps preventing the anticipated competition and forecast savings.
 
Although these are only a few examples, they have rapidly emerged as both the federal and (some) state governments have started to implement ACA provisions. 

Regardless of the political histrionics surrounding “socialized medicine”, the ACA is now (for better or worse) the law of the land. The issue of whether “rationing” exists in any health care system is a red herring; in a world of scarce resources, rationing is performed regardless of political ideology.

The real question is: how will rationing ultimately be conducted- out in the open as part of a rigorous deliberative process, or behind closed doors by legislative fiat?  In this sense, the US politicians’ attack on NICE was both prescient and half-correct:   the ACA would indeed give new power to legislators and bureaucrats to make decisions on the clinical and economic efficiency of medical treatments for patients, and NICE is the (sometimes unpopular) face of those rationing decisions.
 
At the end of the day, however, the implementation of the ACA raises seminal new questions about whether the business of politics can be separated from the dispassionate analysis needed to evaluate clinical treatments.  NICE is far from perfect: the institution has received withering criticism for tough decisions and a less than perfect methodology (e.g., the QALY).  One of the main challenges for US politicians, or the healthcare market, is whether they can come up with a better system.

Thank You!

Hi sweet readers,

Links are coming shortly, but I wanted to thank everyone who wrote in to Mondays kitteh-heavy Ask the Internet. Your suggestions have been invaluable, and in the last four days or so, we:
  • Started feeding Tim Riggins like a nine-pound cat instead of a 35-pound dog. This has already made a Hummer-sized dent in our scooping responsibilities.
  • Are gradually switching him from Target brand litter to Feline Pine. I swear, I could taste the clay dust on my tongue last week, so this has been huge.
  • Combed the pet store, reading ingredient lists on cat food bags. We settled on a small bag of Nutro adult, and will see how he adjusts. (Though, I think well be okay. This cat eats anything vaguely food-shaped.)
  • Gave him a wine cork, milk container seal, and a paper towel tube to play with. Between those, a $2 packet of jingly balls, and the feather-on-a-stick our neighbors left when they moved, hes been pretty happy.
Were still working on a slight nipping problem, but beyond that, its all systems go. Thank you again for helping us go from feline-ignorant to feline-semi-knowledgeable.

Onion Samosas

Onion samosas, love them very much. These onion samosas are my all time favorite snack.I tasted my first onion samosas long back when we went to beach with my parents. My mom will rarely buy finger foods and if she dont bring anything with her while going out, she will definitely get these crispy samosas from a usual onion samosa vendor.These samosas will be very crispy outside and the onions inside will be very crunchy.Thats makes the difference when compared to the other samosas.

With very few ingredients, we can make this samosa in a jiffy for a tea time snack. Kids will love these as they are deep fried and also at the same these samosas will be very crispy and crunchy.This samosas suits pectly for a birthday party, a crowd pleaser, a pect starter, thats why am sharing this beauties today as our this weeks theme for blogging marathon is occasional and my choice is birthday party..Check out the Blogging Marathon page for the other Blogging Marathoners doing BM#27.


Wrap:
1cup Maida 
1/2cup Wheat flour
Salt
2tbsp Oil

Stuffing:
2nos Onions (chopped lengthwise & finely)
1/2cup Flattened rice/poha
1tsp Chilly powder
3tbsp Coriander leaves (chopped)
1/2tsp Garam masala powder
1/4tsp Chaat masala powder
Salt

Paste:
2tbsp Maida
3tbsp water

Take the maida, wheat flour,salt and oil in a bowl, add gradually enough water and knead as soft and stiff dough,keep aside for half an hour.

In an another bowl, take the onions,poha,chilly powder, coriander leaves,garam masala powder,chaat masala powder and salt, mix everything well and keep aside for 20minutes.

Poha will absorb the water from onions,thats why we are adding poha in this stuffing.

Make tiny balls from the dough, dust with  flour and roll them as circles, cut in the middle of the circles.

Make two demi cricles, just fold the sides to make a cone, now gently put some already onion,poha mixture inside the cone.

Meanwhile prepare the maida paste by mixing the flour and water together.

Use this maida paste  to stick the edges..continue the process until the masala and dough get finished..

Heat enough oil for deepfrying, gently drop the samosas, fry until they turns golden brown.

Enjoy with ketchup or with a cup of tea..
