Do Employer Based Wellness Programs Work

Reuters tackles worksite wellness outcomes
Mrs. Smith (name changed) was overweight.  She knew it and her physician, the Disease Management Care Blog, knew that she knew it.  Since the DMCB was one of the two persons on the planet who knew her true weight, she could talk to it behind closed doors about diet, exercise, fads, over the counter meds, prescription appetite suppressants and even bariatric surgery. 

What the DMCB quickly discovered was that Mrs. Smiths weight-loss goals were not only unrealistic but, like many women struggling with weight, driven more by the prospect of how shed look in a bathing suit than any real health benefit.

Mrs. Smith wasnt alone. This seminal study demonstrated just how unrealistic womens weight loss goals (in the range of 50 lbs.) can be.  Think of the popularity of The Biggest Loser and its easy to see why persons think thinness is just a matter of a few months of dieting and exercise, and that being skinny leads to health and happiness, 

Easy, right?

DMCB readers know otherwise. Thats why theyre not going to be impressed by the tone of this May 24 Reuters article on worksite wellness.  RAND, in a not-quite released report to Congress, examined the impact of several employee based programs and found, in the words of the Reuters reporter, only a "modest effect."  Average weight loss was "only" three lbs., tobacco cessation rates were significant but "short term," average cholesterol levels were unchanged and reductions in health insurance claims expense failed to achieve statistical significance.

Researchers have known for years that conservative dietary and lifestyle therapy typically results in weight loss in the range described above. In addition, cholesterol reduction as a primary prevention intervention is low yield when it comes to health. On the other hand, even short term tobacco cessation is a good thing. When it comes to the ability of wellness to reduce health care costs, weight reduction is unlikely to drive claims expense for a health insurer within two to three years, the impact of obesity on overall mortality rates is not as large as youd think and "prevention" rarely saves money.

Whats more, these programs were able to achieve their "modest" outcomes without increasing claims expense.  Participants lost weight and stopped smoking at no additional cost to the system.  Now that is something.

Mrs. Smith and Reuters are very similar.  Both are struggling with nrealistic expectations thanks to dubious fashion trends, media misinformation and scientific ignorance.  Fortunately, Mrs. Smith had access to a resource that could help her better manage her weight.  The DMCB can only hope that Reuters has access to a resource that can help it manage its lack of background knowledge.

The 25 Food Project Day 5

I’m attempting to feed my 6-foot, 205-pound fiancĂ© for one week for $25. This is our story.

I suddenly understand why Ma fed me PB&J sandwiches from kindergarten clear through junior high. Because Day 5, my friends? Was brought to you by peanut butter.

We cleared the 2600 minimum by more than 200 calories, thanks to six tablespoons of Jif Extra Crunchy spread among various dishes. (Er, plus an enormous brunch - picture to the right.) The Husband-Elect loves him some peanut butter, especially on banana bread, so he didn’t mind one bit.

He does mind the lack of grazing. “I feel good,” he says, “but I miss the simple pleasures of snacks and random eating.” When we started this, I didn’t realize how often we take food when we want it, as opposed to when we’re allowed. Now, I have a lot more sympathy for people who don’t have that option.

There are two more days to go, but this is already an eye-opening experience. Looking forward to sharing with yis all on Wednesday.

Substances in household products harm the lungs

Sucking tiny particles or nanomaterial contained in a wide range of household products on the market can cause inflammation and damage to the lungs, according to the study. Products used in the home can include sunscreen, copier ink, or cleaning products.

The study looked at two types of the most widely used nanomaterial in industrial, electronic, or drugs. Nanomaterial commonly used to make the product more robust and flexible.

Eat seven portions of fruit and vegetables for happiness and mental health


Happiness and mental health are highest among people who eat seven portions of fruit and vegetables a day, according to a new report.

Economists and public health researchers from the University of Warwick studied the eating habits of 80,000 people in Britain. They found mental wellbeing appeared to rise with the number of daily portions of fruit and vegetables people consumed. Wellbeing peaked at seven portions a day.

The research was carried out in conjunction with Dartmouth College in the USA and is due to be published in the journal Social Indicators Research.

Most western governments currently recommend ‘5 a day’ for cardiovascular health and as protection against cancer risk.

In Britain today, a quarter of the population eat just one portion or no portions of fruit and vegetables per day. Only a tenth of the British population currently consume the magic number of seven or more daily portions. The study does not distinguish among different kinds of fruits and vegetables and it defines a portion as approximately 80 grams.

Study co-author Professor Sarah Stewart-Brown, Professor of Public Health at Warwick Medical School, said “The statistical power of fruit and vegetables was a surprise. Diet has traditionally been ignored by well-being researchers.”

She emphasized that much remained to be learned about cause-and-effect and about the possible mechanisms at work, and that randomized trials should now be considered.

Fellow co-author, economist Professor Andrew Oswald from the Centre for Competitive Advantage in the Global Economy (CAGE) in the Department of Economics at the University of Warwick, said: “This study has shown surprising results and I have decided it is prudent to eat more fruit and vegetables. I am keen to stay cheery.”

This aloo kachoris turned out super crispy with a wonderful,spicy potato filling. We couldnt stop ourselves having these kachoris, i prepared almost 15 kachoris, for my surprise they just vanished very quickly. You doesnt even need any side dish to have them, the spiciness of the filling itself suits prefectly to enjoy with a cup of tea. Just imagine having this kachori with a cup of masala chai while its raining outside,makes a wonderful evening snacks na. 2cups All purpose flour/Maida
2tbsp Oil
Salt
Oil for deepfrying

For filling:
3nos Potatoes (big & cooked)
1tsp Coriander powder
2nos Green chillies (chopped)
1/4tsp Red chilly powder
1/2tsp Amchur powder
1/2tsp Cumin powder
Few coriander leaves (chopped)
1/2tsp Garam masala powder
Salt

Take the cooked potatoes,coriander powder,chopped green chillies,red chilly powder, amchur powder,cumin powder,garam masala powder,chopped coriander leaves, salt together in a bowl.

Mash and mix everything well.

Make small balls from the potato mixture and keep aside.

Meanwhile knead a soft and stiff dough with maida,oil and water and keep aside for half an hour.

Make medium sized rolls from the dough and roll gently with the rolling pin.

Now place the prepared potato filling balls in the center of the rolled dough.

Cover the filling with the dough by slowly stretching it over the filling.

Seal the ends and remove excess dough,roll gently again with rolling pin.

Meanwhile heat some oil for deep frying,fry the kachoris in batch in medium high flame.

Cook for about 6 to 10 minutes till they gets a golden brown.

Serve and enjoy warm.

Eggless Hersheys Prefectly Chocolate Chocolate Cupcakes

Life is unpredictable,beautiful and precious, can you imagine that you crossed a wonderful human few months ago just like that and now you cant start your day without her.Fortunately its happened to me and she is celebrating her two years of marriage anniversary today, eventhough miles separates us i really want to celebrate this day with her atleast through this virtual world with this eggless chocolate cupcakes.Happy Anniversary my dear Sis, wishing you both many more years of togetherness,happiness,loads of love and many more beautiful things, may god bless you both.

Our this months Baking Eggless, a baking event started by Gayathri entering into its second year after a beautiful year of many eggless bakes.You rock Gayathri,seriously am happy to be a part of this monthly event,our this months challenge is this yummyilicious,very chocolatey and droolorthy chocolate cupcakes, Gaju of Gajus Kitchen suggested this bake and its a big hit among my kids. Actually i couldnt able to bake earlier as i do usually, so many things to finish and a weekend to London kept me busy, to be honest i even thought to skip this challenge. Finally yesterday i finished baking these beauties, oh boy these cuties are really addictive. I replaced the eggs with flax seed meal and obviously its works out wonder. Thanks to both Gayathri and Gaju for pulling us to make this fabulous cupcakes and my family enjoyed it thoroughly.


For cupcakes:
Recipe Source: Hersheys
1+3/4cup All purpose flour
2cups Sugar
1/2cup Vegetable oil
1cup Milk
1cup Boiling waer
2tbsp Flaxseed meal
3/4cup Dutch process cocoa powder
1+1/2tsp Baking soda
1+1/2tsp Baking powder
1tbsp Vanilla essence
1tsp Salt

Heat the oven to 350F, line the muffin mould with paper liners and keep aside.

Stir together the sugar,flour,cocao powder,baking powder,baking soda,flax seed meal and salt in a bowl.

Meanwhile take together the milk,vegetabke oil an vanilla in an another bowl, make a well in the middle of the dry ingredients.

Gradually add the wet ingredients,boiling water to the dry ingredients,mix it well, the batter will be thin.

Pour enough batter to the prepared pan, bake for 20-25minutes until a skewer inserted comes out clean.

Remove the pan from the oven and let them cool completely.


For Chocolate Mascarpone Cheese Frosting:
1/2cup Mascarpone cheese
3tbsp Dark chocolate powder
1cup Powdered Sugar
2tbsp Milk
1tbsp Vanilla
Few sugar pearls

Beat together the cheese,chocolate powder, powdered sugar,milk and vanilla essence until everything get well mixed.

Transfer to the piping bag and pipe it on the top of the cupcakes as you desire.Sprinkle the sugar pearls as you desire.

Just enjoy these beauties with your favourite drink.

Daily Healthy Tips

When it comes to being healthy and fit, its the little things you do every day that make a difference. Here are the things I try to do to stay strong, lean, and full of energy so I can live a long, healthy life. Some are harder to stick to than others (like exercising before work), but when I make an effort, I always feel better. Im curious to know if youre doing them, too, and if youre not, you may learn some new tips to leading a healthier life.

There are a few healthy habits which can be followed in daily routine life. These habits will improve blood circulation in the body and also increases the energy levels and fitness.

Exercise

If you are planning to add a daily dose of exercise to your routine, this is the right time to start. There is nothing like an early morning walk to start with. A good brisk walk of about 30 minutes will recharge your batteries for the day.

Half way round you could also exercise your joints by gently moving them around in different directions. If you are in your 30s, you could also jog for a brief while. People with arthritis must avoid jogging or too brisk walk. Avoid polluted areas for a walk. A couple of rounds in a nearby park will pump your spirits up.

If you are game for company, motivate your neighbors to join. Some like walking alone. Such people can try taking a Walkman along and listen to some good morning ragas.

A good walk in the evenings too is beneficial for health. When exercising outdoors in the winter time, choose all weather, well cushioned flat-soled shoes and thick cotton socks.

Most people have unhealthy breathing habits? They hold their breath or breathe high in the chest or in a shallow, irregular manner. These patterns have been unconsciously adopted, accidentally formed, or emotionally impressed. Certain "typical" breathing patterns actually trigger physiological and psychological stress and anxiety reactions!

While we eat what we like and relish, we also need to fulfill the desires of our tummies. Choosing Wisely Unnecessary Testing and the Simplistic Use of Web 1 0 to Attempt Physician and Patient Behavior Change

There are certain Disease Management Care Blog behaviors that the spouse finds.... objectionable.  She has verbally educated the DMCB about them and has left written notes about them.  Since these communications havent worked all that well, are there any other "channels" she could use? 

The DMCB spouse may be tempted to use the approach of the Choosing Wisely web-site. It provides lists of widely used medical tests and treatments that are typically unnecessary.  The lists (located here) were compiled as part of a joint physician and education project by organizations such as the American College of Cardiology (dont do stress tests in patients without cardiac symptoms) and the American College of Physicians (dont get back x-rays if the ache is non-specific). According to the press reports heralding the availability of this just opened web-site (for example), its purpose is to prompt patients and physicians "to work together" to not only make clinically wise choices, but save the U.S. health care system a lot of money.

The DMCB knows that the academic experts and organizations that developed these lists have been talking and writing about these wasteful tests and treatments for decades.  Waste has been the subject of infinite numbers of continuing education programs, grand rounds, published review articles, national meetings and guidelines.

Those approaches havent worked. 

So is the fix is to repackage the information and post it as simple text on a web page?

The DMCB cant say its impressed.  While the evidence underlying the clinical recommendations themselves is superb, there isnt any evidence that internet-based physician instruction is any better than traditional methods.  And if the purpose of the Choosing Widely web site is to prompt patient behavior change, the evidence that web-based education is better than print is pretty skinny there too. 

Its ironic that the intellectual rigor of the lists themselves wasnt matched by the same evidence-based approach to using the internet for this exercise in the first place.

Whats more, if these groups insisted on using the internet, the least they could have done was deploy a Web 2.0 approach.  The current Choosing Wisely site is so.... has been

That being said, there is some information that could be of use to the population health management service providers.  In contrast to the physician organizations, the DMCB is confident that theyll know how to package and present the information so that it can have a real impact.

The DMCB wishes Choosing Wisely good luck.  And in the meantime, it looks like the DMCB spouse will have to continue to rely on verbal and written feedback.... for now.

No similar link was found for total flavonoid intake in women. Co-lead study author, Dr. Aedin Cassidy noted “These exciting findings provide further confirmation that regular consumption of flavonoids can have potential health benefits.”

This was the first study to examine the connection between flavonoid consumption and the development of Parkinson’s disease. The findings suggest that a sub-class of flavonoids called anthocyanins may exhibit neuroprotective effects. Participants consuming one or more portions of berry fruits each week were around twenty-five per cent less likely to develop Parkinsons disease, relative to those who did not eat berry fruits.

Flavonoids are the bioactive, naturally occurring chemical compounds found in many plant-based foods and drinks. Eating bright colored fruits and vegetables may prevent or delay ALS



New research suggests that increased consumption of foods containing colorful carotenoids, particularly beta-carotene and lutein, may prevent or delay the onset of amyotrophic lateral sclerosis (ALS). The study, published by Wiley in Annals of Neurology, a journal of the American Neurological Association and Child Neurology Society, found that diets high in lycopene, beta-cryptoxanthin, and vitamin C did not reduce ALS risk.

Carotenoids give fruits and vegetables their bright orange, red, or yellow colors, and are a source of dietary vitamin A. Prior studies report that oxidative stress plays a role in the development of ALS. Further studies have shown that individuals with high intake of antioxidants, such as vitamin E, have a reduced ALS risk. Because vitamin C or carotenoids are also antioxidants, researchers examined their relation to ALS risk.

According to the National Institutes of Neurological Disorders and Stroke (NINDS) roughly 20,000 to 30,000 Americans have ALS—also known as Lou Gehrigs disease—and another 5,000 patients are diagnosed annually with the disease. ALS is a progressive neurological disease that attacks nerve cells (neurons) in the brain and spinal cord, which control voluntary muscles. As the upper and lower motor neurons degenerate, the muscles they control gradually weaken and waste away, leading to paralysis.

"ALS is a devastating degenerative disease that generally develops between the ages of 40 and 70, and affects more men than women," said senior author Dr. Alberto Ascherio, Professor of Epidemiology and Nutrition at Harvard School of Public Health in Boston, Mass. "Understanding the impact of food consumption on ALS development is important. Our study is one of the largest to date to examine the role of dietary antioxidants in preventing ALS."

Using data from five prospective groups: the National Institutes of Health (NIH)–AARP Diet and Health Study, the Cancer Prevention Study II-Nutrition Cohort, the Multiethnic Cohort, the Health Professionals Follow-up Study, and the Nurses Health Study, researchers investigated more than one million participants for the present study. A total of 1093 ALS cases were identified after excluding subjects with unlikely food consumption.

The team found that a greater total carotenoid intake was linked to reduced risk of ALS. Individuals who consumed more carotenoids in their diets were more likely to exercise, have an advanced degree, have higher vitamin C consumption, and take vitamin C and E supplements. Furthermore, subjects with diets high in beta-carotene and lutein—found in dark green vegetables—had a lower risk ALS risk. Researchers did not find that lycopene, beta-cryptoxanthin, and vitamin C reduced the risk of ALS. Long-term vitamin C supplement intake was also not associated with lower ALS risk.

Dr. Ascherio concludes, "Our findings suggest that consuming carotenoid-rich foods may help prevent or delay the onset of ALS. Further food-based analyses are needed to examine the impact of dietary nutrients on ALS."

Nutrition Basics

Nutrition can have a tremendous impact on overall health and will be a large part of the focus of the challenge. If you’ve been a Wellness Mama reader for a while, you know that I’m not a fan of grains, sugars and starches and recommend removing them from your diet. Of course, only if you are making sure to get adequate nutrition from proteins, fats and vegetables.
Some foods, like beans, are of questionable nature, so for the duration of the challenge, I’d suggest removing them too! Others, like vegetable oils, margarine, and other frankenfats are toxic and should be avoided completely, challenge or not!
Mini Challenge #1
Cut out the grains, sugars, potatoes, beans and all processed foods from your diet and focus on getting maximum nutrition from meats, vegetables, healthy fats (coconut, olives, coconut oil, olive oil, tallow, lard, butter, unprocessed cream, etc) and fruits. 
Everyone is entering the challenge for different reasons, but if one of your goals is to lose weight or improve your physique, consider drastically reducing your carbohydrate intake.
If weight loss is a serious goal, this will mean that that morning bowl of oatmeal or potato salad at lunch is out. To keep it simple, just get your carbs from vegetables (minus potatoes) and you’ll be fine! One exception, if you struggle from low-thyroid, don’t drop your carbs too low for too long, as this can stress the thyroid, just make sure to get your carbs from healthy foods like sweet potatoes, winter squash, fruit, and grain-free baked goods!
[Note: Some people experience a "carb flu" as their body adjusts to not having a constantly available stream of glucose. This is temporary, but can be uncomfortable for the first couple of weeks. Check out these two posts by Dr. Michael Eades to help make the transition easier

Some Essential Bone Cancer Prevention Tips

Bone cancer may be primary or secondary in nature. If the origination of cancer is in the bone, it is referred to as primary bone cancer. If it will spread to the body from other body parts, it is known as secondary bone cancer. Some examples of primary bone cancer are histiocytoma, sarcoma and Osteosarcoma. Undoubtedly, there are some useful tips to prevent bone cancer. One of them is the awareness of the risk factors. It was suggested by a study that if fluorinated water is consumed by men, they are at a high risk of developing bone cancer. However, girls were not affected by the intake of fluorinated water.   3. Nutrition and diet play a very important role in the prevention of bone cancer. A check must be kept on the intake of calories. A person must stick to a diet that is low-fat. It is concluded by studies that if vegetables and fruits are included in the diet, there is a low risk for developing cancer. The intake of red meat and saturated fat must also be reduced.
  4. Yoga and physical exercise are very effective in curing various medical disorders. The blood circulation is improved and the growth of cancer is limited.
  5. Cancer must be diagnosed before metastasis. Experts must be consulted for detection and screening the condition before it will spread.
  6. One can also follow some herbal tips for preventing bone cancer. The use of ginseng and turmeric must be incorporated in preparing food. There is a compound known as cur cumin which is present in turmeric. Various properties for fighting cancer are present in it.

Knowing the Risk of Prostate Cancer Through Finger

There is no simple formula to determine who will have problems in the prostate. Therefore, many doctors recommend that men over the age of 40 years of a medical examination the prostate.

Another way to determine the risk of prostate cancer, at least, according to research published in the British Journal of Cancer, is to look at differences in finger length. It is said that men who have a longer index finger than ring finger had a lower risk for prostate cancer.

Finger length has been formed before the baby was born and is influenced by sex hormones in the womb. The fetus is exposed to less testosterone will have a longer index finger size and protected against prostate cancer later in life.

"The results of this research may someday be used as a screening tool to be combined with other risk factors, such as genetic testing and history in the family," says Professor Ros Eeles, researchers from the University of Warwick and the Institute of Cancer Research, UK.

The study adds evidence of previous studies that mention the link between the influence of hormone balance in fetal and diseases that may be sustained in the future. It is not impossible if the will of the pregnant women can do in ways of prevention of cancer risk in prospective baby.

Even so, the men who have a shorter index finger does not need to worry because prostate cancer is more influenced by the age factor. The older the age of a man, the higher the risk of having prostate problems.

Acne, Acne Info & How to Overcome Acne | Acne is often a very frustrating problem, and a burden to us, because Ive been there itself was for many years, and honestly once that I was very overwhelmed physically and psychologically, because of the acne problem. However, Alhamdulillah now Ive been able to free from acne problems, acne and scars disappeared, or at least no longer seem annoying too.

I started having acne problems when I was a teenager, about 2 junior class. It was said that this age is often a starting point for acne prone, because it is at these ages are going very significant hormonal changes that trigger the occurrence of acne. At that time my acne was not the worst, still only about 2-3 points on the face, but has begun to interfere with my heart. I start to jab irritated my acne, so finally at a later date my acne problem was getting worse and becoming more and more, almost evenly across my face.

Vaccinations No Need Repeated At Teens

Children who had received hepatitis B vaccine, at the age of 0-5 years, no need to re-vaccinated at the age of 15-21 years. Hepatitis B vaccine capable of protecting the body for 20-25 years. This is true for healthy children and have a normal immune system, and able to form antibodies after vaccination.
Thus the results of research Gunardi Hartono, a pediatrician, for his doctoral dissertation. He examined 213 respondents from a number of high schools and universities in Jakarta. They were aged 15-21 years, and ever received hepatitis B vaccine during childhood.

Hartono defend his dissertation in front of the test team at the Faculty of Medicine, University of Indonesia, Jakarta, Thursday (8/9), and graduated with a GPA of 3.92.

Hartono suggest, infants of mothers who have hepatitis B vaccinated immediately. For children over age 5 year, who have never been vaccinated and are members of families affected by chronic hepatitis B, it must be checked. If he had not been infected with hepatitis B virus, he must be immediately vaccinated. If already infected, immunization is of no use. Their bodies were expected to form antibodies that can resist the virus.

According to Hartono, children aged under one year susceptible to viral hepatitis B. Those who get the virus at an early age at risk of experiencing chronic liver disease which can lead to liver cirrhosis and cancer.

The problem, in children aged under one year, only 10 percent showed symptoms, such as weakness, lethargy, no appetite, eyes and yellow skin, yellow urine and pale feces. Most of the symptoms is unknown so the disease progresses rapidly.

"The sooner attacked by the hepatitis B virus,chronic liver disease the potential becomes even greater, because the immune system has not evolved," he said.

Professor of Pediatrics Faculty of medicine "Agus Firmansyah" Hartono said the promoter, hepatitis B is a serious global health problem. Indonesia including hepatitis B endemic areas with a prevalence of 9.4 percent.
